Eligible Criteria
West Bengal Health and Family Welfare Department and National Urban Health Mission (NUHM) published required eligibility criteria for ANM Course 2019 under UPHC.
Age Limit
Only Female candidates are eligible to apply for the training course. Age of the candidates should be between 25-35 years as on 01/01/2019.
For the candidates belonging to SC/ST/PH category, a 5 years relaxation will be admissible in the upper age limit where else for the OBC candidates, a 3 years relaxation will be admissible.
Resident
the candidates should be a permanent resident of the particular Urban Public Health Centre (UPHC) for which she will be selected.
Educational Qualifications
Minimum educational qualification should be Higher Secondary (10+2) from West Bengal Council of Higher Secondary Education for equivalent examination passed from any recognised Council/ Board.
Other Criteria
The candidate should be a Married or Divorced or Widowed woman to apply for ANM Nursing 2019 UPHC.

UPHC Nursing Job Scope
After completing 2 years ANM Training Course for UPHC under NUHM, the candidates will join to their respective Urban Local Bodies (ULB) / Health Centres by the respective CMOH Officer of their district. The salary of the ANM Nursing staff under UPHC will be approx ₹13,000 per month.
Candidates Selection Process
Candidates selection will be purely on merit, based on the marks obtained by the candidate in the best five subjects (except Environmental Science / Studies) in the Higher Secondary examination or equivalent examination.
Application of candidates from the Vocational Stream will not be considered. The candidate must be medically fit to apply for this training course.

Download the Application Form
At first, you have to download the Application Form, from the official website of West Bengal Health and Family Welfare Department or from our website (link is given below). Take print out the application form.
Fill the Application Form
Now fill the application form with your details information like, the Health Center name for which you want to apply, your name, Date of Birth, Address, Educational Qualifications, HS Marks etc.
Attach Documents
You have to attach the following self attested photocopies of each of the following documents with the Application Form.
- Original copy of Residential / Domicile Certificate issued by BDO, SDO, Local MLA, MP, Panchayat Sabhapati etc
- Age proof Certificate (Admit Card / Certificate of Madhyamik Pariksha)
- Mark sheet of Higher Secondary or equivalent examination.
- Caste certificate in case of SC/ST/OBC Candidates.
- Physically Handicapped Certificate (if any).
- Recent self-attested passport size photograph which is to be affixed on the top right-hand corner of the application form.
Send the Application Form
The application must be sent by Registered post / Speed post / by hand only to the concerned Chief Medical Officer of Health (CMOH) Office of the respective district. The Last Date of receiving application is 26th June 2019.
Candidates have to send their application form to the CMOH office of their own District. Like, applicants of Kolkata will send their application form to the Chief Municipal Health Officer (CMHO), Kolkata.

Merit List
The Merit List for ANM Nursing Course 2019 NUHM will be prepared by the concerned CMOH Office after a week of the last date. The ULB wise merit list will be published on the Notice Board of the CMOH Office. So, contact your CMOH Office for the Merit List.
